usb: Multiple improvements:
* Support varying-sized logical sectors in a single build, instead
of a hardcoded SECTOR_SIZE.
* Properly truncate READ CAPACITY(10) if drive has over 2^32 sectors
And fix potential alignment issues
* Properly truncate READ_FORMAT_CAPACITY if drive has over 2^32 sectors
* Support READ CAPACITY(16)
